When Oncologists Said Elevated Platelets Count After Chemotherapy Was Good!

Jenny (not real name), 53 years old, was diagnosed with left breast cancer in January 2006. A mastectomy was done followed by chemotherapy. After her first chemotherapy on 22 February 2006, Jenny felt weak with a heavy head. She vomited for three days. Jenny was supposed to have 8 cycles of chemotherapy but after her fourth chemotherapy, she decided to stop chemotherapy totally. This was because of the severe side effects.

On 29 November 2006, we interviewed Jenny. Below is the transcript of our conversation.

Question: Okay, when you did your second, third and fourth chemo, did the doctor know you were on herbs?

Jenny: No. I hinted about taking herbs during my consultation. They said: “No, no, no. Don’t take herbs.” But I took it quietly without their knowledge. After the fourth chemo and it was time for the fifth chemo, I called and informed them that I intended to stop the treatment. I told them that I am a Christian and I believe in God’s healing. I heard from God to stop the chemo. And I said I wanted to go for herbs. Initially the oncologist said, “Okay, it’s your choice. Do whatever you want. If you have any problems you can come back here.” I accepted that.

Jenny’s Platelet Count Went Up

Jenny: Every few weeks I had my blood test done before this. Every blood test, the blood platelets kept going up. My friends told me through their experiences the blood platelets usually go “down” not “up”. If it is “up” it is not good. Also the red blood cells must go up, white blood cells must go up but the blood platelets must come down. When I saw my blood platelets went up, I asked the oncologist. He told me, “No, it is okay. All must go up. Then your body is responding to the chemo.” So, I was very happy and continued with my second, third and fourth chemo. The platelets went up higher and higher until it exceeded the range. So, for the third and fourth chemo, the platelets already exceeded the range. I asked the oncologist: “The platelet had already exceeded the range! Is it dangerous?” The doctor replied, “No, it’s good! It must go up”.

One month after I stopped the chemotherapy, I went back to him again for a check up. They did a blood test and found that the platelets count was dropping. The red blood cells and the white blood cells were up, but the platelets came down. Seeing this I panicked because the oncologist said the platelet count must go up. Initially, when I entered the room, the oncologist said: “You are doing very well. You are okay.” But as we went through the readings of the blood and came to the part on platelets count, from 600 it dropped to 300, I asked him, “How? Why it dropped so much? Last time you said 600 is good. Now, it has dropped to 300 . What is wrong with this? Is it okay?” Then he said, “Well, you go for herbs. That is why it is like this. You go for organic food, you believe in God’s healing – that is why it is like that! What organic food you are talking about. What is organic food?” I was very confused. I felt like crying. I had no one to consult.

I went to have another test done at the Wellness Laboratory. Yes, the platelets dropped to half the amount! I enquired from the technician about this drop in platelet count. He told me it was good for me. Eh? I was dumbfounded. There were two opposing views and I didn’t know whom to believe. Then I waited until you (Dr. Teo) came and spoke to you. Then, you clarified that it was good that the platelet count went down.

Question: Your impression was that you were doing badly after the blood test showed a drop in the platelet reading?

Jenny: Yes.

Comment:

With regards to the platelet count, when the oncologist said: It is good. It must go up! The body is responding to the chemo, he was both deadly wrong and also right. He was deadly wrong because a rise in platelet count or thrombocythemia, may cause clotting in blood vessels and abnormal bleeding. Abnormal clotting of the blood is a more common complication than abnormal bleeding. A blood clot can occur in either an artery or less frequently, in a vein. This complication can be very serious if the clot blocks blood flow to an organ, such as the brain (causing a stroke) or heart (causing a heart attack). The oncologist was absolutely right when he said abnormal elevation of platelets was the body’s response to chemotherapy except that it is NOT for good, it could be deadly!

What is real sad about this case is that the oncologist did not tell his patient the truth! He misled her – either deliberately or through sheer ignorance!

October was Breast Cancer awareness month and Physical Therapy awareness month. What better way to celebrate and raise awareness by continuing to give the gift of fitness to survivors by working with one group of professionals best trained to work with them, physical therapists, and by encouraging everyone you love to make fitness a regular part of their daily lives.

Everyone knows of someone affected by Breast cancer. Over 271,420 new cases of Breast cancer were expected to be diagnosed in 2005. Breast cancer survivors have very special needs requiring a skilled professional at least until they reach a maintenance phase in exercise programming.

Once their physician provides a medical clearance exercise a woman can begin exercising. Exercise programs provide flexibility exercises for the affected arm and improve postural alignment and awareness after surgery.

Some of the exercise benefits of participating in a regular exercise program for Breast Cancer survivors are:

- increased mood
- increased strength and endurance
- prevention of complications after surgery and treatment

Exercise helps to battle the side effects of treatment and aid in Breast Cancer recovery. Additional benefits of exercise include improvements in function and pain perception and all psychological areas according to the latest research. Exercise will help with daily activities and prevent further weakness and musculoskeletal problems.

Once full range of motion or mobility are restored strength training for the affected arm can begin. While working towards full range of motion, continue strengthening those body parts that are not affected. You’ll need the strength in the rest of your body to be functional and efficient with your daily activities. Some women on hormonal therapy are at risk for developing osteoporosis. Weightbearing exercise can help offset any possible fractures.

Physical activity is still possible even with lymphedema, a chronic swelling of the affected arm when lymphatic fluid collects and is unable to drain properly. Close supervision by a Physical Therapist or Occupational Therapist is a good idea to receive proper guidance for exercise selection and instruction. Not only can strengthening occur but the therapist can assist in developing alternate routes for lymphatic fluid drainage to prevent the likelihood of developing lymph edema. Therapists can also show you how to monitor your arm if you are prone to developing lymphedema. The risk of lymphedema is also decreased by participation in a supervised exercise program. Physical therapists are well trained and qualified to provide evidence-based, supervised and individualized exercise programs for willing survivors. An integrative and complete exercise program will focus on strength and flexibility of the shoulder, arm, back and hand in addition to posture, aerobic endurance, lower body training and stress reduction.

Exercises for core strengthening and stabilization are another way to improve function and strength. Proper postural retraining is very important as changes tend to occur following surgery which if left unaddressed can lead to the development of other problems.

Deep breathing techniques will assist with controlling and returning lymphatic fluid back to the heart By practicing deep and efficient breathing techniques, postural alignment will be improved and stress can be reduced.

Flexibility exercises will increase range of motion where limitations exist not only in the upper body but in the back as well. A flexibility program will also address any scar tissue that may have developed following surgery.

If exhaustion and fatigue are a factor, participating in appropriate exercises throughout the course of the day as opposed to a continuous exercise session may be more tolerable. Cardiovascular or aerobic exercise will boost your endurance. Try something simple and safe initially like walking.

Learn how to use and incorporate the use of your affected arm in your daily activities. This is a great way to functionally work on flexibility and strength.

Remember we can all do something active and encourage those around us to do the same. Exercise is an inclusive activity that we all can benefit from with proper guidance. Controversies in Radiation Therapy for Breast Cancer

Non-invasive breast cancer, also known as ductal carcinoma in situ, or DCIS, is highly curable. It is most commonly detected on a screening mammogram. The standard of care is surgical removal, followed by breast radiation therapy, in women who wish to conserve their breast. The alternative is a mastectomy. Both modalities have equal survival outcomes. However, there has been a great deal of controversy about the necessity for radiation therapy for DCIS in women who opt for breast conservation.

Proponents for radiation therapy underscore the fact that the risk of a recurrence in the breast can be a more aggressive or even an invasive recurrence. Half of these recurrences are due to invasive breast cancer. The latter has the potential to spread to lymph nodes of the underarm and to other parts of the body. Without radiation therapy, the risk of a local recurrence is approximately 20%. By adding radiation therapy, the risk of such a failure is reduced by half. Needless to say, this is a substantial benefit.

The experts who do not support the routine use of radiation therapy for DCIS espouse that there is no difference in long-term survival between women who undergo radiation therapy versus those who do not. A woman who is willing to accept the higher risk of a recurrence in the breast may be treated with surgical removal alone. The question then arises: which women with DCIS should undergo radiation therapy and which should be observed?

Since the potential common side effects from radiation therapy, such as a skin reaction and fatigue, are temporary and the woman’s risk for developing an invasive and potentially life threatening breast recurrence is significant, it is prudent for women to take the more aggressive approach.

Certain factors, such as the woman’s age, her overall health, the surgical margin (or rim of normal breast tissue removed around the DCIS, with the wider the margin being more favorable), and the grade of the DCIS (low grade is much less aggressive than high grade and intermediate grade falls between these two extremes), should factor into the decision making.

Radiation therapy should be strongly considered for young women, women who have close margins, women with DCIS measuring more than ½ centimeter (cm) and/or those do not have low grade DCIS. On the other hand, studies show that even older women with DCIS measuring less than ½ cm, who have wide margins and/or low grade DCIS, benefit from the addition of radiation therapy.

The good news is that radiation therapy is a very easy, painless treatment that can enable women with DCIS to be at low risk of having the disease return in the treated breast, and peace of mind that they left no stone unturned.

Cancer Preventing Qualities of Green Tea

One of the most important discoveries resulting from the many years of cancer research is the indication that there may be several natural ways to prevent cancer. Learning how we might prevent cancer could be the most important tool in eradicating this disease.

Prostate Hyperplasia: A Brief Explanation

It has been found that almost every man over the age of 60 will develop one of the numerous conditions associated with the prostate gland. Many of these conditions tend to exhibit only mild symptoms until they are well developed. This means that if you are affected, you may not even realise that you have a problem.

One of such conditions is known as Prostate hyperplasia, also known as Benign Prostate Hyperplasia (BPH). Although it is not cancerous, it can cause the same prostate symptoms as prostate cancer. Prostate hyperplasia is present in about 90% of men over the age of 80. However, unless it causes the prostate gland to become grossly enlarged the symptoms are relatively mild and attributed to the rigors of old age.

The cause of prostate hyperplasia is not accurately known. Many researchers and oncologists believe that it is a hormone related condition. The male hormone testosterone is converted to a secondary hormone called dihydrotestosterone naturally in the body and when this secondary hormone binds with specific receptors in the tissues of the prostate gland, cellular growth and division becomes over-stimulated. Thus the prostate gland becomes enlarged so producing prostate hyperplasia.

Certain families of drugs can be used to reduce the amount of testosterone in the body or to stop the available testosterone from binding with the receptors in the prostate gland. This acts to stop prostate hyperplasia from developing further and may even reduce the size of the prostate gland over time. Unfortunately drug therapy for prostate hyperplasia is an ongoing therapy and if diagnosed with the condition you will be required to take drugs daily for the rest of your life.

Prostate hyperplasia symptoms are very similar to those of prostate cancer. Do take action if you notice the following:

* Difficulty urinating
* Urinary leakage
* Pain when urinating
* A feeling of urgency to urinate
* Increased night time urination
* Any other problems associated with urinating

As mentioned above prostate hyperplasia is usually easily controlled and treated with simple drug regimes. But if the symptoms are due to cancer, then surgery may be required.

Still you should not imagine the worst. A diagnosis of prostate hyperplasia does not necessarily mean that you are going to have prostate cancer. Admittedly some cases of prostate hyperplasia do progress and become cancerous.
